Complete the following steps for establishing a new service.
Complete the Sewer Service Application (Application fee will be added to first month's bill.)
Please download the application form and return via email to INSERT NEW EMAIL HERE or postal mail to INSERT NEW PO ADDRESS HERE
Complete the Release of Water Consumption Form and Customer Agreement Form (Notary is required);
Print the Sewer Service Application, Release of Water Consumption Form and the Customer Agreement form. Sign and Fill in all 3 including notary. Mail the original completed documents to:
